Please complete and submit this form to register and pay for the MC test center fee. Once submitted, you will be contacted with instructions to make the $30 payment. 

Payment in full is due before scheduling your test appointment. 

If you plan to take more than ONE CLEP exam, please complete one registration form per exam.
