Institutional Record Tracking of Student Complaints

This is an internal form used to document student complaints that reach the Dean’s or the Director’s level in writing. Concerns that are referred back to the Department Chairs do not need to be recorded. Because of confidentiality issues, this form will only collect general information. However, departments should always retain detailed records.
